Single assessment process

The National Service Framework for older people, published in 2001, introduced the Single Assessment Process (SAP). Full implementation of this process was required by April 2004. Single Assessment is a system of care management covering assessment, care planning service delivery and review.

Single Assessment Process is standard two, the process means that health and social care professionals will work together to deliver better services to adults and older people. It will promote better care services and better outcomes for older people, prevent duplication of information and effective sharing of information between health and social care professionals.
