Equipment quality and outcome questionnaire

We want to improve our services so we need to find out your views about the process and what effect the equipment or adaptation to your home has had on your life.

If we have contacted you by letter because you have received some equipment or a minor adaptation to your home through Bedfordshire County Council Adult Services or on our behalf by another organisation between September and December last year, please tell us your views by completing the on-line questionnaire below.
